GEO vs. SEO: What’s the Difference?
GEO vs. SEO explained for healthcare marketers. Learn how AI visibility differs from traditional search and why strong brands need both.

For years, the dominant search model was fairly straightforward: someone searched Google, Google returned a list of results, the user selected one, and the website earned the visit. Generative search adds another layer to that process. A person can now ask a complex question and receive a synthesized response that draws from multiple sources, cites publishers, compares options, and continues into follow-up questions without requiring the user to begin another traditional search.
That changes the visibility problem for brands. Healthcare marketers are no longer competing only for a blue-link ranking or the first organic click. They increasingly need their organizations, experts, services, data, and content to be understood well enough that search systems can use them when constructing an answer.
That is the basic idea behind Generative Engine Optimization, or GEO.
The distinction is useful, but it needs to be understood correctly. In 2026, Google made its position increasingly clear: from Google Search’s perspective, optimizing for generative AI experiences such as AI Overviews and AI Mode is still SEO. Those experiences continue to rely heavily on Google’s existing ranking, indexing, and quality systems.
So the real question is not whether healthcare organizations should choose SEO or GEO. The better question is how a strong search strategy needs to evolve when the search result itself can now generate the answer.

The biggest mistake is treating the GEO column as though it makes SEO obsolete. In many cases, GEO is better understood as an extension of good SEO into a different kind of search interface.
What Is SEO?
SEO stands for search engine optimization. At its core, SEO is the work of improving a website’s ability to be discovered, understood, indexed, and surfaced through search.
That includes familiar areas such as technical accessibility, site architecture, internal linking, content quality, search intent, authority, structured data, local relevance, and page experience. Traditional SEO reporting also tends to focus on rankings, impressions, organic clicks, traffic, conversions, calls, leads, and revenue.
Those metrics still matter. In fact, Google’s current generative-AI guidance reinforces that traditional SEO fundamentals remain important because AI Overviews and AI Mode still retrieve and rank information using Google Search infrastructure.
That means a healthcare organization does not suddenly need to abandon technical SEO, internal linking, service-page development, author expertise, or content architecture because AI-generated answers exist. If anything, those fundamentals become even more important because generative systems still need clear, reliable, accessible information to retrieve and synthesize.
What Is GEO?
GEO stands for Generative Engine Optimization. The term was formalized academically in research published by Aggarwal and colleagues at KDD 2024, where the researchers described generative engines as systems that synthesize information from multiple sources into generated responses and proposed GEO as a framework for improving source visibility within those responses.
Traditional SEO often asks whether a page can rank for a query. GEO adds another question: can the information on that page be understood, retrieved, cited, summarized, or used when an AI system constructs an answer?
That difference becomes easier to see with a healthcare example. A treatment center may rank well for a search such as “what is PHP treatment?” Traditional SEO evaluates whether the page ranks, earns clicks, and converts visitors.
GEO looks at a broader scenario. What happens when someone asks an AI system, “What is the difference between residential treatment, PHP, and IOP, and which one might make sense after detox?” That is not simply one keyword. It is a contextual, comparative question that may require several sources, multiple concepts, and a synthesized answer.
Generative search increasingly works around relationships between concepts, comparisons, follow-up questions, and context rather than isolated keyword phrases. That does not make keywords irrelevant. It changes how narrowly marketers should think about them.
For Google, GEO Is Still SEO
This is worth stating plainly because a large amount of AI-search marketing already makes GEO sound more mysterious than it needs to be.
Google’s current guidance acknowledges terms such as Generative Engine Optimization and Answer Engine Optimization, but its position is that optimization for Google’s generative AI search features is still part of SEO. AI Overviews and AI Mode use technologies such as retrieval-augmented generation and query expansion, but those systems remain tied to Google’s broader indexing, ranking, and quality infrastructure.
That means healthcare organizations do not need an entirely separate Google content strategy built around speculative AI hacks. Google has also said that special llms.txt files, special AI-only markup, Markdown versions of pages, or artificial content “chunking” are not requirements for appearing in its generative search features.
The practical recommendation is much more familiar: make the site technically accessible, create genuinely useful content, demonstrate expertise, build authority, organize information clearly, answer real user needs, and avoid mass-producing low-value pages.
The interface has changed. The need to be a good source has not.
ChatGPT Search Has a Different Technical Discovery Layer
Google is only one generative search environment. ChatGPT search adds another discovery system with its own technical considerations.
OpenAI states that public websites can appear in ChatGPT search and recommends that publishers who want their content to be discovered, surfaced, cited, and linked allow OAI-SearchBot to access public content. That creates a legitimate platform-specific consideration for GEO.
A website could theoretically be accessible to Google while blocking OpenAI’s search crawler. For organizations serious about AI discovery, technical audits may therefore need to look beyond Googlebot and ask whether relevant search crawlers from other platforms can access public content as well.
That said, crawl access should not be confused with authority. Allowing OAI-SearchBot does not make a website more trustworthy, more useful, or more likely to be selected for a particular answer. It simply makes the site eligible to be accessed.
Crawlability gets you into the pool. It does not make you the best source.
GEO Changes the Meaning of Visibility
Traditional organic visibility is relatively easy to understand. A page ranks, receives impressions, earns clicks, and generates sessions. Generative search can create a less direct path.
Imagine someone asks an AI system, “What should I look for when comparing addiction treatment centers?” The response might synthesize information from multiple healthcare websites and cite one treatment provider’s educational resource.
The user may not click immediately. They may remember the organization, search the brand later, send the information to a family member, or return days later through a completely different channel.
The original AI exposure may therefore influence the eventual conversion without appearing as the last-click source.
That is why AI visibility needs its own measurement layer, but it should not become the next vanity-metric problem.
Google Is Beginning to Make Generative Visibility More Measurable
Google has continued adding reporting around its generative search experiences, giving marketers more visibility into how pages appear in AI-driven search environments.
That progress is useful because it provides first-party data instead of forcing marketers to rely entirely on third-party screenshots and manually repeated prompts. But it also creates a familiar temptation. A healthcare executive may soon receive a report saying that AI visibility or generative impressions increased dramatically.
The next question still needs to be: what did that visibility produce?
Did branded search increase? Did the website receive qualified referral traffic? Did more people search for the organization by name? Did those visitors become qualified opportunities? Did admissions or revenue move?
Visibility matters, but visibility is not ROI.
GEO Should Not Become the Next Vanity-Metric Industry
SEO has spent years teaching executives to celebrate metrics such as keyword count, traffic, impressions, rankings, domain scores, and screenshots. Many of those metrics are useful, but they become misleading when they are treated as business outcomes.
GEO could easily repeat the same mistake with AI citations, ChatGPT screenshots, AI share of voice, brand mentions, and generative impressions.
Those metrics tell you whether the brand is appearing. They do not automatically tell you whether appearing is valuable.

The technology changes. The business principle does not.
The Best GEO Content Is Often Better SEO Content
One of the most important implications of current AI-search guidance is the growing value of distinctive information.
Generative AI has made average content extremely cheap. Almost any company can now produce a technically acceptable article titled “10 Benefits of SEO for Healthcare Companies” or “What Is Addiction Treatment?” in a matter of minutes.
That content may be accurate, but it is increasingly interchangeable.
The stronger opportunity lies in publishing information that has a reason to exist beyond filling a keyword gap. That can include original healthcare marketing data, real admissions-funnel analysis, anonymized case studies, proprietary research, firsthand operational experience, documented experiments, expert commentary, unique frameworks, and nuanced information that is difficult to reproduce without actual subject-matter knowledge.
AI made writing cheaper. It made original information more valuable.
That is true for SEO, and it is true for GEO.
Clear Answers Still Matter
Generative systems need information they can understand, but human readers need the same thing.
A healthcare article should answer the question being asked without forcing the reader through several paragraphs of vague brand language first.
If the heading is:
What Is a Partial Hospitalization Program?
the section should explain what a PHP is near the beginning.
It should not open with a paragraph about a facility’s “commitment to individualized healing journeys” and wait several hundred words before defining the service.
Clear writing is useful for traditional search, generative search, and patients themselves. The goal is not to write strangely because an AI might read the content. The goal is to make the information easy to interpret correctly.
Build Around Meaning, Not Endless Query Variations
AI search also makes it tempting to create a separate page for every possible conversational variation of a question.
For example:
- What is PHP?
- What does PHP mean?
- What is PHP treatment?
- How does PHP treatment work?
- What happens in PHP?
- What is a partial hospitalization program?
- Is PHP outpatient treatment?
Those are all valid questions, but they do not necessarily justify seven URLs.
A better page may answer the entire topic comprehensively, including what PHP is, who it may be appropriate for, how it differs from residential treatment and IOP, what the daily structure looks like, how housing may or may not fit into the program, and what happens before or after PHP.
That gives the page more depth and creates a stronger resource for both traditional search and generative answers.
Topical completeness is generally more useful than manufacturing endless near-duplicate query pages.
GEO Increases the Importance of Entity Clarity
Search systems do not only need to understand individual pages. They increasingly need to understand the organizations, people, services, and locations behind those pages.
For healthcare organizations, that means basic facts should be clear and consistent. A website should make it easy to understand who the organization is, where its facilities are located, which services are directly provided, which services are handled through referral relationships, who the clinicians and leaders are, which levels of care are offered at each location, and who authors or reviews clinical content.
If one page says the organization provides medical detox while another says detox is referred externally, that inconsistency creates a problem long before AI gets involved. Patients, families, referral sources, search engines, and AI systems are all receiving conflicting information.
Good GEO often starts with something much less glamorous than an AI optimization tactic: make the truth about the organization obvious everywhere.
GEO Is Also a Brand and Authority Problem
Traditional SEO became heavily page-focused. Marketers often ask what a particular URL ranks for, which keywords moved, and how many links the page has.
Generative discovery introduces a broader question: what does the web understand this organization or expert to be?
A company saying on its own website that it is an expert is one piece of evidence. That expertise becomes more credible when it is also reflected through original research, conference appearances, interviews, professional profiles, cited publications, podcasts, industry participation, videos, and legitimate third-party references.
This is where digital PR, founder visibility, subject-matter expertise, and brand authority become more directly connected to search.
A strong entity is easier for both people and machines to understand.
Healthcare GEO Requires a Higher Standard
Healthcare is not a low-stakes category.
Someone may ask an AI system whether alcohol withdrawal can be dangerous, whether detox should come before residential treatment, how PHP differs from IOP, or whether a family member might need a higher level of care.
Inaccurate answers can influence real health decisions.
Healthcare organizations should therefore approach GEO content with the same seriousness they would apply to any other patient-facing health information. That means maintaining clinical accuracy, using strong sourcing, clearly identifying authors and reviewers, accurately describing services, avoiding exaggerated treatment claims, and regularly updating important information.
The objective should not simply be to become citable.
The objective should be to become worth citing.
Does Schema Help GEO?
Structured data can help machines understand webpage information when the markup accurately represents visible content. That makes schema useful infrastructure.
It should not be treated as an AI-ranking shortcut.
For Google specifically, the company has repeatedly pushed back against unsupported GEO hacks and special AI-only requirements. A page with perfect schema but generic, inaccurate, or low-value content is still generic, inaccurate, or low-value content.
Structured data can clarify meaning. It cannot manufacture expertise.
Does GEO Mean Keywords No Longer Matter?
No. Keywords still provide valuable information about how people describe problems, services, and questions.
They can reveal demand, intent, commercial relevance, terminology, and content gaps.
What changes is how literally marketers should treat them.
A healthcare marketer should not only ask whether a site can rank for “residential treatment Tennessee.” The better strategy also asks what someone researching residential treatment actually wants to understand.
That may include questions such as:
- Who needs residential treatment?
- How is residential different from PHP?
- Does detox need to happen first?
- Will insurance cover care?
- How long does residential treatment last?
- Can someone travel from another state?
- What happens after discharge?
Those questions create a more useful resource and help the organization address the actual patient decision rather than simply targeting one phrase.
How Should Healthcare Organizations Measure GEO?
Start with visibility metrics, but do not stop there.
AI visibility can include generative impressions, cited URLs, ChatGPT or other AI-search referrals, brand mentions, recommendation presence, and share of voice where reliable data is available.
The next layer should evaluate engagement. Track which pages receive AI referrals, whether those users engage, whether branded search changes, and whether people return through other channels after being exposed to the brand.
Then connect those signals to business outcomes such as qualified opportunities, sales conversations, admissions, revenue, and ROI.
OpenAI states that ChatGPT search referral URLs can include utm_source=chatgpt.com, which gives publishers at least one direct way to identify some ChatGPT-originated traffic. That is useful, but it still will not capture every AI-influenced patient journey.
A person may see a brand in an AI response, never click the citation, and search for the organization later. That is why GEO measurement will likely require a combination of referral analytics, branded-demand analysis, CRM attribution, and broader business outcomes.
SEO Fights for Discovery. GEO Expands the Fight Into the Answer.
The most useful distinction is not that SEO and GEO are two entirely separate disciplines. It is that the search environment has expanded.
Traditional SEO asks whether someone can discover the organization through search.
GEO adds another question: can the search system understand the organization well enough to use its content, expertise, or brand when constructing the answer?
Sometimes that creates a click. Sometimes it creates a citation. Sometimes it creates brand familiarity that influences a later visit.
That makes attribution more difficult, not less important.
Healthcare Brands Need Both Because They Are Increasingly Part of the Same System
The wrong response to AI search is to stop doing SEO and chase GEO as though the fundamentals no longer matter.
The equally wrong response is to assume AI search changes nothing and keep operating exactly as marketers did five years ago.
Search is evolving, but the core requirements remain remarkably consistent. Technical accessibility matters. Content quality matters. Authority matters. Brand matters. Original information matters. Accurate service descriptions matter. Expertise matters.
What changes is how those signals are surfaced.
Google may use them inside AI Overviews or AI Mode. ChatGPT may retrieve and cite them through search. Other generative platforms may use them when producing recommendations or comparisons.
That is why the strongest strategy is not SEO versus GEO.
It is building a search presence strong enough to survive the interface change.
The Goal Is Not to Rank in AI. The Goal Is to Be Chosen.
This is where GEO has to reconnect with business strategy.
A healthcare organization does not ultimately need more AI citations simply for the sake of being cited. It needs more of the right people to discover the organization, understand what it provides, trust the information, consider the program, contact admissions, qualify appropriately, and enter care when the service is a fit.
A citation can contribute to that. An AI impression can contribute to that. A traditional organic ranking can contribute to that.
None of them is the final outcome.
SEO and GEO are both parts of acquisition infrastructure. The business result still sits downstream.
